print all 2 pages
fill out ALL relevant sections A - J
sign section D & E
provide all copies of relevant material requested including a colour copy of photo ID as stated in section K
email all documentation to: admin@coomerarealty.com.au
OR fax to: (07) 5556 0899

*PLEASE NOTE: if faxing application, a colour copy of ID must still be sighted and copied for file.
